Agreement/Disclaimer
·
I
agree, in consideration of a formal contract between Wazee Sports
Center and myself, to assume all responsibility for the rental
equipment listed below from the date and time of said rental until
its return to Wazee Sports Center.
·
I
agree to assume all responsibility and charges, at prevailing rate,
for the repair and/or replacement of any or all equipment damaged,
lost or stolen while in my personal use or possession.
·
I
agree to examine the equipment thoroughly before I leave the
facility to ensure its proper working condition.
·
I
agree to hold and save Wazee Sports Center and all employees
harmless from any loss or liability in the failure or malfunction of
the specified equipment while in my possession. By signing
this form, I understand that I give up my right to sue in the event
of any accident or injury, including injuries caused by equipment
malfunction.

I
vouch that I have adequate knowledge and/or experience in the use of
said rental equipment and will follow whatever safety and legal
regulations that apply to standard and prudent diving practices.
·
I
agree that the specified rental equipment is for my sole and
personal use, and not for sublet or any other use by anyone else.
A nationally recognized certification card in the bearer’s name must
be displayed before the release of tanks and regulators unless the
renter will be under direct and continual supervision of a certified
instructor.
·
All equipment is rented on the basis of that day’s daylight hours
and must be returned and rinsed by closing time of the rented day
unless other arrangements have been made. Delay in return will
result in additional rental costs.
Please note:

A driver’s
license and certification card must be provided at the time of
rental.
¨
No cash
refunds on rental gear or refunds given on unused cylinders.
¨
Be certain to
carefully examine the equipment before leaving the dive store.
¨
All equipment
must be paid for in advance.
¨
NOTE:
Wisconsin law requires the use of a white on red
divers flag.
Signing below
indicates that I have read and agree to the above rental agreement
and I have also inspected all my equipment and reported any
discrepancies to dive shop employees.
